Well the no regerating health should have been obvious from the demo.
Health doesn't regenerate. You have to use medipacks to recover health.
Medipacks have to be crafting using the crafting system (Looting/item scavenging is a big part of the game). However, the same materials for a medipack can be used to make a molotov cocktail, so you essentially have to choose between recovering your health or having an option for offense/defense. Depending on what you choose to do with your items, different things may happen.
Using the molotov vs medipack example, if you choose to make the medipack then you may find yourself out of ammo in the middle of a fight. Now you have no offensive options other than your fists, whereas if you had saved your items to make a molotov you would be able to fight back. Now you're essentially a sitting duck against 2 or 3 armed guys. Now say you made the molotov, you may have used it early in the fight and now you're low on health and you've still got to kill 2 or 3 guys.

Not open-world, open-ended.
As in, there's multiple ways to approach a situation.
Like in the demo they just went in guns blazing but during an interview the lead designer said that they could have bypassed that entire section by sneaking past the enemies and up/down the stairs.
